Small teams, clear ownership
Departments are small enough that you will know what happens on either side of your own work. Quality has the final word on release, and nobody is asked to sign something they are not comfortable signing.
Training is structured: you are assessed against the procedures for your area before you work unsupervised, and re-assessed when anything changes. Movement between functions is common — several of our QA staff started in production.
